Tribologische Optimierung von Hüftendoprothesen durch Entwicklung einer künstlichen Synovialflüssigkeit und eines neuartigen Hüft-Tribometers
Laufzeit: 01.06.2023 bis 01.06.2025

In Deutschland entscheiden sich jährlich rund 200.000 Menschen für eine künstliche Hüfte und lassen ihr Hüftgelenk durch eine Endoprothese ersetzen. Die Operation gehört zu den Standardeingriffen in der orthopädischen Chirurgie. Die Hüftprothese ist während der Bewegung, insbesondere die Gleitpaarung zwischen Hüftkopfkugel und Pfanne, starken Belastungen und daher auch einem starken Verschleiß ausgesetzt. Durch mechanische Arbeit der beiden im Gelenk befindlichen Reibpartner wird ein nicht unerheblicher Materierialabrieb erzeugt, welcher zu einer Entzündung des umliegenden Gewebes und einer nachfolgenden Lockerung der Endoprothese führen kann und ein Hüftprothesenwechsel erfordert. Der Hüftprothesenwechsel wird allerdings erschwert, weil die Endoprothese sich im Vergleich zur ersten Operation schlechter im Knochen befestigen lässt. Ursache hierfür ist Entzündung verursacht durch den Metall - und Plastikabrieb der Gleitpartner im künstlichen Gelenk, welche zu einer Abnahme der Knochensubstanz führen. Die Schmierflüssigkeit eines gesunden Hüftgelenks ist die von der Gelenkschleimhaut produzierten Synovialflüssigkeit. Durch Implantation und Gelenkentzündung verändert sich die Zusammensetzung dieser Synovialflüssigkeit, was zu einer verminderten Schmierfähigkeit der Flüssigkeit führt. In dem hier beantragten Vorhaben sollen die Eigenschaften der Synovialflüssigkeit vor und nach Prothesenimplantation näher untersucht werden. Mit Hilfe der gewonnenen Ergebnisse soll eine künstliche Synovialflüssigkeit hergestellt werden, welche den Anforderungen der optimalen Schmierung des Implantats gerecht wird. Zur Untersuchung der tribologischen Eigenschaften soll ein neuartiger Prüfstand entwickelt werden, der als Hüftkopfkugel/Pfanne-Prüfstand ausgeführt ist und die Reibung unter Berücksichtigung einer realistischeren gelenkähnlichen Schmiermittelmenge (ca. 3-10 ml) direkt an der Prüfpaarung messen kann. So wird es möglich für jede Gleitpaarung eine optimale Schmierflüssigkeit zu identifizieren und zu prüfen wie die künstliche Synovialflüssigkeit der humanen Synovialflüssigkeit gleicht und welche Einflussfaktoren die Schmiereigenschaften positiv beeinflussen. Ziel ist es, eine künstliche Synovialflüssigkeit mit optimalen Schmierbedingungen zu entwickeln, welche potentiell in Zukunft im Patienten appliziert werden kann.

Abgeschlossene Projekte

Abaksi- Antibakterielle Silber-Beschichtung von Endoprothesen
Laufzeit: 01.04.2021 bis 01.04.2023

Der gestiegene Bedarf an Gelenkimplantaten hat zu einem erhöhten
Auftreten periprothetischer Gelenkinfektionen geführt, die häufig
durch Bakterien der Staphylococcus spp. Familie verursacht werden.
Bisher eingesetzte Antibiotika sind nur in der direkten Phase nach der
Implantation wirksam. Um bakterielle Infektionen langfristig zu
unterdrücken, müssen Lösungen für eine lang anhaltende
antibakterielle Wirkung etabliert werden. Dafür kommt der Einsatz von
Silber auf der Implantatoberfläche in Frage. In diesem Projekt soll
untersucht werden, wie das Silber durch Funkenerosion mit
beigemischten Silber-Nanopartikeln in Dielektrikum (Powder-mixed
EDM - PMEDM) in die Randschicht des Implantats während der
formgebenden Bearbeitung eingebracht werden kann. Eine
durchgehende und vergleichsweise dünne Legierungsschicht (˜ 2,5
µm), welche unter anderem mit Silber angereichert ist, wurde in
Vorversuchen hergestellt und hinsichtlich ihrer antibakteriellen
Wirkung bewertet. Bei einem Silbergehalt von 3,78% wurde eine
signifikante Verringerung der Anzahl der Bakterien und Cluster von
Staphylococcus aureus erzielt, während ein signifikanter Anstieg an
Osteoblasten-Zellen im Bereich von 0% bis 4,84% Silbergehalt
beobachtet wurde. Die Homogenität der Silber-Abscheidung in der
Randschicht stellt jedoch bei PMEDM für Implantatanwendungen
weiterhin eine große Herausforderung dar. Darüber hinaus muss die
Effizienz der antibakteriell-biokompatiblen Eigenschaften der
modifizierten Oberflächen über einen längeren Zeitraum bewertet
werden. Die übergeordneten Ziele des Projekts sind daher die
Beherrschung des PMEDM-Prozesses zur gezielten Erzeugung
solcher definierter Randschichten und die Gewinnung eines vertieften
Verständnisses der antibakteriell-biokompatiblen Wirkung von
TiAl6V4-Implantatoberflächen, die mit Silber angereichert sind. Der
zentrale Ansatz der Forschung ist die gleichmäßige Verteilung von
Silber-Nanopartikeln im PMEDM-Bearbeitungsspalt durch Anwendung
einer optimalen Spülstrategie mit Unterstützung von Multiphysik-
Simulationen und Ultraschallschwingungen. Darüber hinaus werden
die durch PMEDM-Bearbeitung erzeugten Langzeiteigenschaften
hinsichtlich antibakterieller Wirkung und Biokompatiblität der
Randschichten bewertet.

Die Funktion von BCP Kristallen in der Regulation des chondrozytären Phänotyps in der Arthrose
Laufzeit: 01.03.2020 bis 28.02.2023

Die Kalzifizierung des Knorpels mit basischen Kalziumphosphat (BCP) ist ein häufiger Befund bei Arthrose (OA) und hängt direkt mit dem Schweregrad der Erkrankung zusammen. Ich habe in einer früheren Studie herausgefunden, dass die BCP-Verkalkung direkt mit der hypertrophen Differenzierung von Chondrozyten zusammenhängt. Ich fand typische Anzeichen für OA-Knorpelveränderungen bei stark verkalkten ttw / ttw-Mäusen, die ein natürliches OA-Mausemodell sind. Die OA in ttw / ttw-Mäusen war von einer erhöhten perizellulären Matrixsulfatierung begleitet. Das gleiche Sulfatierungsmuster wurde in menschlichen OA-Knorpelschnitten beobachtet. Immunhistochemische Färbung zeigte eine Anhäufung von WNT3a um Chondrozyten mit zunehmendem OA-Schweregrad, die mit Heparitinase entfernt werden konnte, was auf eine Bindung von Wnt3a an Heparansulfat-Proteoglykane (HSPG) hindeutet. Ich fand eine Erhöhung der 6-O-Sulfotransferase-Expression im menschlichen OA-Knorpel. In Übereinstimmung mit diesem Befund war die ß-Catenin-Färbung mit steigendem OA-Gehalt im Knorpel von Mäusen und Menschen erhöht. Zur Erklärung dieses Phänomens fand ich heraus, dass BCP-Kristalle Wnt3a binden und kanonische WNT-Signaling induzieren. Dieser Effekt könnte durch den extrazellulären Wnt-Inhibitor DKK 1 blockiert werden. BCP-Kristalle induzieren eine hypertrophe Differenzierung der Chondrozyten mit erhöhtem Kollagen X und MMP 13, sowie herunterreguliertem Sox 9 und Aggrecan. Ich gehe davon aus, dass BCP-Kristalle Wnt3a in der perizellulären Matrix konzentrieren, wodurch die Verfügbarkeit von Wnt3a erhöht wird. Die Chondrozyten neigen aufgrund der erhöhten 6-O-Sulfatierung von HSPGs dazu, mit zunehmendem OA-Schweregrad kanonische Wnt-Signale zu induzieren. Die BCP-Mineralisierung extrazellulärer Matrix ist kein Epiphänomenon, sondern ein aktiver Schritt bei der weiteren Aufrechterhaltung der Chondrozyten-Differenzierung bei Osteoarthritis.
Folgende Fragen sollen in dem Antrag untersucht werden:
1.) Wie interagieren BCP-Kristalle auf Rezeptorebene mit Chondrozyten? Ist der LRP-Rezeptor oder die HSPGs wichtig? Ist die negative Ladung von HSPGs für das Wachstum der Mineralisierung wichtig? Können die gleichen Veränderungen bei negativen HSPGs während der endochondralen Knochenbildung beobachtet werden?
2.) Welche intrazellulären Wege werden durch BCP-Kristalle aktiviert? Ist der nichtkanonische WNT / Ca2 + / CamKII-Weg für den BCP-Effekt verantwortlich? Sind andere Ca2 + -abhängige Wege an der BCP-induzierten Chondrozytendifferenzierung beteiligt? Wie induzieren BCP-Kristalle Veränderungen im Sulfatierungsmuster von HSPGs? Ist die Induktion von HS6ST1 von Ca2 + abhängig? Sind Änderungen in HS6ST1 altersabhängig?
3.) Wie wird die Bindung von Proteinen an die Oberfläche von BCP-Kristallen vermittelt? Liegt es an der Lipidierung von sekretierten Proteinen? Kann die Auflösung von BCP-Kristallen oder die Änderung der HSPG-Sulfatierung den Wnt3a-Effekt beseitigen?

ROR2 blockade for cartilage regeneration and pain relief in osteoarthritis
Laufzeit: 01.01.2020 bis 31.12.2022

We have discovered that ROR2 blockade, using RNAi, induces chondrogenesis resistant to hypertrophy. In therapeutic regime, ROR2 blockade results in reduced cartilage destruction and sustained pain relief in a murine model of osteoarthritis induced by joint destabilization. With our current technology, ROR2 blockade requires intra-articular injections of siRNA conjugated with atelocollagen every 5 days. Frequent intra-articular injections are not acceptable in routine clinical practice. We intend to develop ROR2 blockade which can be administered systemically or intra-articularly not more often than every 3 months, and biomarkers predicting efficacy. In aim 1 we will generate blocking reagents such as a monoclonal antibody or soluble extracellular domain of ROR2. Such reagents will be tested and validated in human chondrocytes in vitro as well as in vivo in murine models of osteoarthritis. In aim 2 we will chemically stabilise siRNA and optimize carrier molecules to achieve efficient ROR2 blockade by systemic injections or persistence in the joint after intra-articular injections lasting at least three months. In aim 3 we will use transcriptional targets of ROR2 signalling as markers predictive of response to ROR2 blockade and /or as surrogate potency markers. Such markers will be useful for patient stratification and rapid outcome assessment.

ABINEP M3-project 3: Investigation of biofilms during septical prosthesis relaxation
Laufzeit: 01.04.2017 bis 28.02.2022

Die hier beantragte ESF-geförderte internationale OVGU-Graduierten- schule (ESF-GS) Analyse, Bildgebung und Modellierung neuronaler und entzündungsbe- dingter Prozesse (ABINEP) soll die Ausbildung internationaler Promovierender in den be- sonders forschungsstarken Profillinien der Medizinischen Fakultät der Otto-von-Guericke- Universität (OVGU) unterstützen und ausbauen. Die durch diese ESF-GS geförderten OVGU-Profillinien sind die Zentren für Neurowissenschaften (CBBS) und für die Dynami- schen Systeme (CDS, einschließlich Immunologie/Molekulare Medizin der Entzündung). Die ESF-GS umfasst 4 thematische Module mit insgesamt 21 Stipendiaten, die den o.g. Schwerpunkten z.T. parallel zugeordnet sind und die organisatorisch unter dem zentralen Dach der ABINEP ESF-GS zusammengefasst werden sollen. Jedes der 4 thematischen Mo- dule wird mit 5-6 Stipendiaten ausgestattet. Die Module, die Zuordnung der Anzahl der Stipendien und die durch sie unterstützten OVGU-Forschungsstrukturen sind unten aufgeführt. Weiterhin sind die inhaltlich eingebundenen außeruniversitären Partner benannt:

  • 1. Neuroinflammation ( 5; CBBS, CDS, OVGU, FME, LIN, DZNE)
  • 2. Modellierung neuronaler Netzwerke ( 5; CBBS, OVGU, FME, LIN, DZNE)
  • 3. Immunoseneszenz ( 6; CDS, FME, HZI)
  • 4. Bildgebung menschlicher Hirnfunktionen ( 5; CBBS, OVGU, FME, LIN, DZNE)

Die CBBS-assoziierten Module weisen eine starke Vernetzung mit den Ingenieur- wissenschaften (v.a. dem Transferschwerpunkt Medizintechnik) auf, die über eine unab- hängig beantragte eigene ESF-GS (MEMoRIAL) gefördert werden sollen. Eine enge Koope- ration zwischen diesen beiden ESF-GS ist geplant, um Synergien sowohl in der Ausbildung der Stipendiaten als auch für innovative neue Forschungsansätze in Zusammenarbeit mit dem Transferschwerpunkt Medizintechnik der OVGU und dem Landesprojekt Autonomie im Alter zu erreichen. Insgesamt fördert die ESF-GS ABINEP die Internationalisierung der anerkannten exzellenten medizinischen Forschung der OVGU.

Untersuchung der zellulären Prozesse der Chondrokalzinose
Laufzeit: 01.06.2018 bis 31.05.2020

Das Ziel dieser Studie ist die molekularen Mechanismen der Entstehung von CPPD-Kristallen und ihre Wirkung auf die Chondrozyten des Gelenkknorpels besser zu verstehen. Ein besonderes Augenmerk liegt hierbei auf dem Prozess der zellulären Seneszenz, dem bisher hauptsächlich durch Untersuchungen an Mausmodellen eine wichtige Rolle in der Pathogenese der Osteoarthritis zugeschrieben wurde. Um dieses Ziel zu erreichen sollen folgende Fragen untersucht werden:

1.) Wie verhält sich die Menge der CPPD-Kristalle zum Osteoarthrosegrads des Patienten mit CC?

2.) Welcher Zellphänotyp (Seneszenz, Inflammation, Hypertrophie) ist mit der CPPD Synthese verbunden?

3.) Welchen Effekt haben CPPD-Kristalle auf den Chondrozytenphänotyp in vitro?

4.) Welche Therapievarianten der Chondrokalzinose kann es geben?

Regulation der phänotypischen Stabilität von Chondrozyten durch transmembrane Heparansulfat-Proteoglykane der Syndecan-Familie
Laufzeit: 01.05.2013 bis 30.04.2018

Verschiedene Studien haben gezeigt, dass transmembrane Heparansulfat-Proteoglykane der Syndecan-

Familie eine Vielzahl unterschiedliche Proteine binden können und dadurch unterschiedliche Funktionen in
Zellen haben können. Syndecan-4 (Sdc-4) ist insbesondere während Streßsituationen wie Frakturheilung,
Wundheilung und Osteoarthrose (OA) von Bedeutung. In meinen vorhergigen Studien habe ich gezeigt, das die Funktion von Sdc-4 während der Embryonalentwickung durch Sdc-2 kompensiert werden kann, jedoch nicht in Streßsituationen wie OA und in der Frakturheilung. Darüber hinaus habe ich gezeigt, dass Sdc-4 in der WNT-Signaltransduktion eine Rolle spielt, welche in beiden Prozessen, OA und Embryonalentwicklung, von entscheidender Bedeutung ist. Basierend auf meinen bisherigen Daten stelle ich die Hypothese auf, dass Sdc-2 und -4 eine entscheidende Rolle in der Regulation der phänotypischen Stabilität und Differenzierung von Chondrozyten spielen. Ich möchte daher in meinem hier beantragten Projekt auf die Mechanismen der WNT induzierten Signaltransduktion über Syndecane, insbesondere Sdc-2 und-4, fokussieren. Darüber hinaus möchte ich die Rolle von WNT-Signalwegen in der Regulation der phänotypischen Stabilität von Chondrozyten in vitro und in vivo unter Verwendung von Wildtyp- und Sdc-defizienten Mäusen untersuchen, und ihre Funktion in der OA. Die Ergebnisse dieses Projektes werden dazu beitragen die pathogenetischenMechanismen der OA besser zu verstehen und den Weg ebnen neue therapeutische Ansatzpunkte für diese Erkrankung zu entwickeln.

Mechanosensitive signalling complexes as early targets of post-traumatic osteoarthritis (OA)
Laufzeit: 01.01.2015 bis 31.12.2017

Der artikuläre Knorpel dient insbesondere der Reduktion von mechanischem Stress während der Bewegung. Einige Studien legen nahe, dass eine Überbelastung des Knorpels zu einer spezifischen Antwort der Chondrozyten führt, die dann in einer Osteoarthrose endet. Es bleibt jedoch die Frage offen, wie Chondrozyten den mechanischen Stress spüren und wie dies zu dem Verlust der phänotypischen Stabilität der Chondrozyten führt. Unsere Hypothese lautet, dass mechanosensitive Signalkomplexe, bestehend aus Adhäsionsmolekülen und membrangebundenen Kalziumkanälen, entscheidende Mediatoren und somit auch therapeutische Targets in der OA sind. Neue Studien aus unserer Arbeitsgruppe haben gefunden, dass das Oberflächen- Moleküle Syndecan-4 (Sdc-4), die bei Verlust oder durch Blockade die Induktion von OA verhindert. Weitere Studien haben gezeigt, dass Sdc-4 mit Kalziumkanälen der TRPC-Familie interagiert, welche mechanosensitiv sind. Wir vermuten daher, dass die mechanosensitiven Signalkomplexe aus Sdcs und TRPC-Kanälen bestehen. Basierend auf unseren Daten planen wir, mittels gut etablierter Methoden sowohl molekularbiologisch, als auch mit zellbiologischen Methoden das Zusammenspiel und die Funktion von Sdcs (insbesondere Sdc-1 und -4) mit TRPC-Kanälen (insbesondere TRPC1 und -6) in vitro und in vivo und im Tiermodell der OA zu untersuchen. Ein Fokus wird auf der Analyse der Signalwege unter mechanischem Stress liegen, die zu einem Verlust der phänotypischen Stabilität von Chondrozyten führen. Hierfür werden TRPC Aktivatoren und Inhibitoren, sowie knock out Mäuse verwendet werden. Das Zusammenspiel von Sdcs mit TRPCs und die zugrunde liegenden Signalwege werden in Doppel- knockout Mäusen in vitro und in vivo untersucht. Zusammenfassend soll dieses Projekt das therapeutische Potential der Blockade von TRPC Kanälen und Sdcs zur Prävention und Behandlung von mechanisch induzierten Veränderungen im Phänotyp der Chondrozyten aufklären.

Prof. Dr. rer. nat. Jessica Bertrand, Jahrgang 1978, wurde zum 12. Oktober 2015 auf die W2-Professur für Experimentelle Orthopädie an der Otto-von-Guericke-Universität berufen. An der Orthopädischen Universitätsklinik wird sie künftig den Forschungsbereich Experimentelle Orthopädie leiten.
Der Schwerpunkt der Forschungsarbeit von Prof. Bertrand richtet sich auf die Untersuchung der molekularen Mechanismen der Knorpel-Remodellierung unter physiologischen und pathologischen Bedingungen. Dies umfasst die Mechanismen der enchondralen Ossifikation und pathologischen Prozesse, wie Osteoarthrose, Frakturheilung und rheumatoide Arthritis. Die Kernhypothese ihrer Arbeit ist, dass es zu einer erneuten Aktivierung embryonaler Signalkaskaden kommt, die sowohl während der Frakturheilung, als auch der Osteoarthrose aktiv sind und denen der enochondralen Ossifikation in der Embryonalentwicklung in großen Teilen gleichen.
